ABSTRACT

Introducción: se realizó un análisis de la evidencia disponible sobre intervenciones orientadas a mejorar la adherencia al lavado de manos, para generar recomendaciones para los centros de atención en salud ambulatoria.Materiales y métodos: se realizó una búsqueda en Pubmed, Embase y Epistemonikos, y en las referencias de guías sobre el tema, seleccionando aquellos estudios no observacionales en que se realizaran intervenciones para aumentar la adherencia al lavado de manos en atención en salud. Se clasificaron las estrategias descritas según tipo de intervención, y se evaluaron según su efectividad en mejorar la adherencia al lavado de manos y el seguimiento en el tiempo.Resultados: se seleccionaron 34 estudios experimentales prospectivos, que se clasificaron en 10 grupos según el tipo de intervención realizada, y se evaluaron según efectividad y seguimiento en una escala del I al VII. 24 de 34 estudios mostraron un aumento estadísticamente significativo de la adherencia al lavado de manos mayor al 15% desde el basal o en comparación al grupo control, correspondiendo en su mayoría a estrategias multimodales y de feedback. Discusión: las intervenciones más efectivas para mejorar la adherencia al lavado de manos fueron las que incluían abordajes multimodales y las que incluían feedback. Se hace urgente generar mayor evidencia sobre esta temática en el contexto de atención ambulatoria.


Introduction: To analyze the available evidence on interventions aimed at improving adherence to handwashing, to generate re-commendations for outpatient health care centers. Methods: A search was made in Pubmed, Embase and Epistemonikos, and in the references of guides on the subject, selecting those non-observational studies in which interventions were carried out to increase adherence to handwashing in health care. The strategies described were classified according to the type of intervention, and evaluated according to their effectiveness in improving adherence to handwashing, and its follow-up time afterward. Results: 34 prospective experimental studies were selected, which were classified into 10 groups according to the type of intervention performed, and were evaluated according to effectiveness and follow-up on a scale from I to VII. 24 of 34 studies showed a statistically significant increase in handwashing adherence greater than 15% from baseline or compared to the control group, corresponding mostly to multimodal and feedback strategies. Discussion: The most effective interventions to improve adherence to handwashing were those that included multimodal approaches and the ones that included feedback. It is urgent to generate more evidence on the subject in the context of ambulatory care.

Objetivo Intervenções psicossociais têm tido impacto positivo na vida das pessoas com transtorno mental grave, porém tais intervenções devem ser implementadas fielmente ao protocolo. A fidelidade refere-se à medida que uma intervenção adere ao modelo original e sua avaliação é essencial para que os desfechos possam ser creditados à intervenção. O objetivo deste estudo foi realizar uma revisão sistemática da literatura mundial das intervenções psicossociais destinadas a pacientes com transtornos mentais que possuem um instrumento ou método de avaliação de fidelidade ao modelo original. Métodos Pesquisas bibliográficas sistemáticas foram realizadas para encontrar estudos relevantes ao tema nas seguintes bases de dados: Embase, Medline, Scopus e SciELO. Foram incluídos estudos de intervenções psicossociais, realizados na comunidade, dirigidos a pacientes diagnosticados com transtornos mentais. Intervenções de comparação poderiam incluir tanto o tratamento-padrão como uma intervenção de comparação ativa. A qualidade dos estudos foi avaliada de forma independente por dois revisores, utilizando critérios adaptados de instrumentos validados. Resultados Trinta estudos preencheram os critérios de inclusão. Os estudos mostraram a eficácia da avaliação da fidelidade em diferenciar diferentes modelos de tratamento, sua validade preditiva para os desfechos e a confiabilidade dos instrumentos utilizados, bem como os fatores facilitadores e os obstáculos para a obtenção de alta fidelidade nas intervenções avaliadas. Objective Psychosocial interventions have had a positive impact on the lives of people with severe mental illness. However, such interventions should be implemented accurately according to the protocol. Fidelity refers to the extent that an intervention adheres to the original model and its evaluation is essential so that the outcomes can be credited to the intervention. This study aims carry out a global systematic review of psychosocial interventions to patients suffering from mental illness which have an instrument or assessment method of fidelity to original model. Methods Systematic literature searches were performed to find studies relevant to the theme in the following databases: Embase, Medline, Scopus and SciELO. Studies of psychosocial intervention, conducted in community care for patients diagnosed with mental disorders were included. Comparison interventions could include either the standard treatment or intervention of active comparison. Study quality was assessed independently by two reviewers using criteria adapted from validated instruments. Results Thirty studies met the inclusion criteria. Studies have shown the effectiveness of the assessment of fidelity to differentiate different treatment models, their predictive validity for outcomes and reliability of the instruments used, as well as facilitating factors and obstacles for achieving high fidelity in the evaluated interventions. Conclusion More than documenting adherence to the original model, fidelity also provides information relating to the target population and the expected outcomes, which allows performing necessary adjustments during its course to be achieved excellence in the implementation process of such interventions. .

ABSTRACT

BACKGROUND: Nutrition delivery is frequently interrupted or delayed by physicians' ordering patterns. We conducted this study to investigate the effect of physician compliance with tube feeding (TF) protocol on the nutritional and clinical outcomes in acute lung injury (ALI) patients. METHODS: After implementing a TF protocol, 71 ALI patients with mechanical ventilation (MV) for > or = 7 days were observed. A dietician assessed the nutritional status of the patients and established individualized nutrition plans according to the protocol. If the physicians followed the dietician's recommendation within 48 hours, the patients were classified under the compliant group (Group 1). RESULTS: Forty patients (56.3%) were classified into Group 1. Prealbumin was comparable in both groups at ICU admission but higher in Group 1 at the time of discharge from the ICU (228 +/- 81 vs 157 +/- 77 mg/dl, p = 0.025). Nitrogen balance was only improved in Group 1. The time to reach calorie goal was shorter and non-feeding days were reduced in Group 1. The proportion of parenteral nutrition to nutritional support days was lower and delivered calories on the 4th and 7th day of TF were higher in Group 1 (p < 0.001). ICU mortality/stay and hospital mortality failed to show differences but hospital stay was prolonged in the noncompliant group (Group 2) (p = 0.023). Arterial oxygen tension and PaO2/FiO2 were maintained during the 1st week of ICU stay in Group 1 but were decreased in Group 2. CONCLUSIONS: Physicians' compliance with the TF protocol contributed to the likelihood of nutritional improvement and a shorter hospital stay in ALI patients with prolonged MV.
